A Team Approach to Personalized Care and Guidance

We  provide services to individuals suffering with mental illness in combination with other contributing factors that decreases their ability to maintain independence and stability in their community.

Re-Entering the Community from the Department of Corrections , Psychiatric Hospitals and other Facilities ,Aging out Juveniles age 18 and older of Court Service Units and or Social Service Involvement ,Veterans and Individuals

  • Interpersonal and Social Skills

  • Medication Management

  • Mental Health Symptoms Management

  • Personal Health and Safety

  • Physical and Nutritional Health

  • Service Coordination

  • 24 hours / 365 days  Crisis Support

  • Community Coordination

 Admission Requirements
  • Adults 18 years of age and older

  • Are diagnosed with a qualify in mental  illness

  • Have been prescribed psychotropic medication in the last 12 months    (even if not taking it)

  • Have been hospitalized for crisis services,TDO, or other intensive services

  • Have significant impairment in major life activities—problems with daily living skills

  • Need  skill-building interventions to maintain community stability and independence

  • Have difficulty with interpersonal relationships,isolation,and the inability to recognize personal danger or inappropriate social behavior

  •  Have received repeated interventions from mental health services, social services,or the judicial system Pre-Screening Assessments
